Grant Application Upper Mississippi Gaming Corporation Revised December, 2006 GRANT GUIDELINES AND GENERAL INFORMATION The original plus 9 copies, all stapled, of this application must be in our office by 5:00 p.m. on the deadline date. TYPES OF SUPPORT AWARDED Capital funds, matching funds and extraordinary funding requirements. AUTHORIZATIONS OR CERTIFICATIONS If any organization is applying for funds for any program/project which involves property that is not owned/operated by the applicant, i.e., a PTA, individual school, etc., the applicant must provide authorization from the owner/operator of the facility (in this case, the school administration). This would include city properties or properties or facilities owned/operated by other agencies. It would also include any documentation certifying an agency to perform any activity, etc., granted by another agency or unit of government. Proof of authorization from the school or government body must be documented with a copy of minutes showing Council or Board approval and the grant application must be signed by the Board President or mayor. The Upper Mississippi Gaming Corporation retains the right to amend this application, or change the process in which applications and/or grants are administered at any time, for any reason. Applicants must use the grant form for the current period, or the application will not be considered. Applying for a grant does not guarantee the requested project or program will be funded. The Board may, at its discretion, choose to fund your request in full, in part, with stipulations, or not at all. Revised December, 2006 GRANT EVALUATION Grant Review Meetings 2008 - March 24 at 6 PM and TBA Proposals are evaluated by the Upper Mississippi Gaming Corporation Board of Directors, who may seek additional information from the applicants or members of the community knowledgeable to the subject area. Final decisions on all grants are made by the Upper Mississippi Gaming Corporation Board of Directors. Grant applications will be rated on demonstration of need, leverage of other funds or community support, amount of matching cash, community impact, proposed evaluation of results, and strength of organization. | INSTRUCTIONS FOR APPLICANTS READ CAREFULLY & COMPLETE All ITEMS.
All items must be completed on the form provided, and TYPED in the space provided. No applications will be accepted that are handwritten or that are on altered forms. Answer all questions. An answer of N/A will not be accepted. Items 1-10 on the cover page provide a quick summary for our reviewers and MUST APPEAR AS THE FIRST PAGE OF THE APPLICATION. Item 11 describes the project or program for which the applicant is seeking funding. Please use only the space provided. No attachments will be accepted. Sections 1 & 2 provide reviewers with more in-depth information about the organization, how the organization will assess the success of the program or project and publicize the funding received from UMGC. Sections 3 - 5 give reviewers information about the program or project, other funding that may be pending, received or denied, past receipt of UMGC funds and the budgets associated with the project or program, the applicant organization, and if applicable, the budget from the governing organization that owns the property involved. Please provide information about reserve funds held by all entities involved in the project/program for which funding is requested. Use attachments as designated in Section 5. Page 8 contains space for listing the Board of Directors and Officers of your organization. If the application is for school or government projects you should list the School Board members or Mayor and Council of the entity. Also on page 8 is a signature line for use only by those who are requesting funds for which approval from a governing body must take place prior to application. A Mayor or Board president's signature is required along with a copy of meeting minutes stating approval of the project or program by the full board or council. The last signature line requires the signature of the person who is submitting the application on behalf of the organization or political sub division. Both signature areas must be completed for a political sub-division application. Page 9 contains the checklist of items that must be included with the application. It is the applicant's responsibility to make sure all information is provided, and that the proper procedure has been followed for submission of the grant application. Failure to provide all requested information will result in disqualification of the application. Notice to applicants: If funded, the organization will be required to sign a grant agreement, which may contain stipulation of funding, or if funded in part, a list of items for which the funds were awarded. Funds must be spent within 6 months unless a request in writing for an extension is granted, or funds must be returned to UMGC. The agreement will also require completion of a grant evaluation form within 90 days of the completion of the project /program. No funds will be released without a signed grant agreement.
